At Dentally we have created a Consent form template that allows you to use an Estimate style template and add your own wording. This allows your patients to digitally sign the consent form through the iPad and online through the Patient Portal.

At Dentally we know who busy you are, and we have several ways to help you save time and effort, including being able to create templates for those forms that you need to create over and over again. One of these is your consent form template.


To locate your Templates go to Settings > Templates>Estimates.

We STRONGLY recommend that you start by duplicating the existing Consent style template and making changes as required. This will create a template that does not display the treatment costs and details, and only shows the wording you enter, along with the patient signature area.

Create a New Consent Template from an existing template

  • To create a new estimate Template, select New Template.

  • Give your template a name.

  • Select Copy existing Template and select Consent Style from the list. This will add all the formatting without you having to edit the Styles.

Because it is a template, all of the code for a consent form is already included.

If you need to change the style of the template, please contact the Dentally Support team. We recommend that you do not try and edit the template style.

Add your consent wording in the Footnote Section

  • To add your own consent working, scroll down to the section called Footnote.

  • This section is simple text.

  • Delete what is on the screen

  • Add your own text from your existing Consent form. You can simply copy and paste it from your existing consent forms.

  • Save and Preview to make sure that it looks as you expect.

Add your Terms & Conditions in the Terms Section

  • In the Terms section, add in your Terms and Conditions

  • This section is simple text so copy your existing terms and conditions. You can simply copy and paste it from your existing consent forms.

  • Save and Preview to make sure that it looks as you expect.

Remove a Consent Template

  • You cannot delete a template;

  • You set a template to inactive

  • When a template is inactive it no longer appears in the list of available templates

  • If you do not wish to use a particular template any more, you can remove it from the list of available template options by going to the Settings > Templates menu, editing the Consent Template and setting its 'Active' status to 'No'.
