How patients can sign their medical history

Take a look at what patients will see when signing their Medical History on the Portal

If you have enabled Medical Histories for your practice. You can now let patients sign this from their device, wherever they are. This saves your reception team valuable time.

Once the patient has logged in. They will see a checklist at the top of the home page. Patients will see a checklist task to sign a Medical History on the portal if they have an upcoming appointment and the patient has not previously signed a Medical History in the last 14 days.

Clicking the card will take the patient to the form to complete the questions and sign. After signing the form, the patient will be taken back to the home page.

Patients are encouraged to answer all the questions. If they have missed out on any questions, the app will not let them proceed until they have done so. If they answer yes to any questions, they are then asked to add additional information which is displayed in Dentally.

What if my patient needs to update an answer?

If a patient has previously completed a Medical History and needs to change an answer, the patient can click on the 'Update your medical info' tile at any time and change their answers.
